29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: A. Tool Room: On, or about, December 2, 2015, a relocatable power tap used to energize a shop-made tool grinding machine. The relocatable power tap was energized through an extension cord. B. Shop Floor, bench provided for Machine #72 Cutmax2 Wotan Boring Mill: On, or about, November 2, 2015, a fan that was not designed for use in an industrial location was used for general ventilation. Also, the fan was energized through a Woods relocatable power tap that was energized through an extension cord. Reno Machine Company, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA Inspection Number 314402769, Citation Number 2, Item Number 2 and was affirmed as a Final Order on January 31, 2011, with respect to a workplace located at Newington, Connecticut.
