29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. A log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completed for 2011 in the detail as required by the regulation. a. At the facility located at 107 Heritage Drive, Pataskala, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2011, a description of work-related injuries and illnesses. On or about April 14, 2011, an employee injured her wrist but there is no description of which wrist was injured. b. At the facility located at 107 Heritage Drive, Pataskala, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2011, a description of work-related injuries and illnesses. On or about April 17, 2011, an employee injured a knee but there is no description of which knee was injured. c. At the facility located at 107 Heritage Drive, Pataskala, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2011, a description of work-related injuries and illnesses. On or about May 24, 2011, an employee injured a knuckle but there is no description of which knuckle on which hand was injured. d. At the facility located at 107 Heritage Drive, Pataskala, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2011, a description of work-related injuries and illnesses. On or about October 7, 2011, an employee injured his hand but there is no description of which hand was injured or what caused the injury. e. At the facility located at 107 Heritage Drive, Pataskala, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2011, a description of work-related injuries and illnesses. On or about November 28, 2011, an employee injured a hand but there is no description which hand or of what specifically she was doing when injured. Abatement certification required within ten days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
