29 CFR 1904.29(a): A Log of all Work-Related Injuries and Illnesses (OSHA Form 300), and/or the Summary of Work-Related Injuries and Illnesses, (OSHA Form 300-A), and/or the injury and Illness Incident Report (OSHA Form 301) or equivalent were not kept by the establishment: a) On or prior to September 13, 2011, Conifer Care Communities dba Christopher House, as the exposing employer, did not used an adequate OSHA 300 Form. The OSHA 300 Form (or equivalent) used by the employer did not contain the information required to be recorded under Column (G); Total number of days away from work. There was no Column (G) on the employers forms for the following calendar years: 2008, 2009, 2010 and 2011.
