29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: On or about 4/23/2015, at 1530 Joy Lane, Decatur, GA; employees working from the fabricated frame scaffold were exposed to fall hazards of 11 feet, 6 inches to the lower level where a scaffold was erected with deficiencies including but not limited to, not fully decked, no access/egress, no side and end rails, no firm footing. The employer failed to insure that the scaffold was inspected prior to use. B&C Masonry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(f)(7), which was contained in OSHA inspection number 314210717, citation number 1, item number 4 and was affirmed as a final order on 8/1/2011, with respect to a workplace located at 845 Scenic Highway, Lawrenceville, GA 30045.
