29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person(s): There was no frequent and regular inspection program being maintained in use by a competent person knowledgeable of regulations and requirements to ensure the job site, materials and equipment such as, grounded electrical extension cords, breaker box openings were enclosed without exposed live electrical parts, extension cords were not spliced or spliced in a compliant manner, compliant single point scaffold systems independent lifeline and scaffold suspension line anchorage points, prompt rescue from fall hazards, compliant systems for warming flammable liquids and fixed ladder fall protection usage were being maintained and utilized in a safe condition.
