29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) ADA Dash Main Line A900; On or about June 17, 2016 and at times prior thereto, Employee #15 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter a robotic cell where there were energized robots and other jigs and fixtures to replace a welding tip on a robot and allowed to clear sensor faults without locking out the electrical, hydraulic and pneumatic energy sources.
