29 CFR 1910.266(i)(3)(ii): Safe use, operation and maintenance of tools, machines and vehicles the employee used or operated, including emphasis on understanding and following the manufacturer's operating and maintenance instructions, warnings and precautions. On or about November 27, 2013, at the job site located on Sugarloaf Tower Road near Calico Rock, Arkansas, the employees who were repairing a Timberjack Model 840 tree feller/buncher, had not been trained on the safe maintenance of the machine. An employee was replacing a hydraulic hose on the equipment with the cutting head elevated five feet above the ground. Once the hose was removed, the cutting head fell on another employee working on the equipment. This condition exposed the employee to the hazard of being crushed by the falling head assembly.
