29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2): In the Storage Yard, Storage Plant, and Assembly Plant, employee(s) engaged in pallet manufacturing operations and material handling activities were exposed to struck by, laceration, and amputation hazards in that the employer did not verify through a written certification that a workplace hazard assessment had been performed whereby employee(s) were working near pallet stacks that reached 16-17 feet in height and were working near and around machinery that could cause lacerations and amputations.
