29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) do not cover the actions listed in and are not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A) Morral Companies, LLC, located at 132 Postle Avenue, Morral, OH 43337: The employer does not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of the hazardous energy sources, are performed. Employees do not de-energize and lockout mechanical hazardous energy sources of the Lawn Mixer, located in the Liquid Department, prior to conducting cleaning/unjamming activities inside the Lawn Mixer. Employees are exposed to mechanical hazards. The violation occurred on or about April 16, 2020, at the above address. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] are not implemented to protect employees from thermal, atmospheric, and mechanical hazards while employees are conducting cleaning/unjamming activities inside the Lawn Mixer located in the Liquid Department.
