29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them: (a) On or about May 28, 2019 - Facility, employees were exposed to struck-by and electrical hazards when the machine specific lockout/tagout procedures for equipment such as, but not limited to, the Pack Mack Bagger, Cutting Vat Dump, and the Bag Take Away Conveyor did not specify the order that locks/devices should be removed or specify that the locks/devices could be removed in any order.
