29 CFR 1910.178(l)(6): The employer does not certify that each operator has been trained and evaluated as required by this paragraph (l): The employer does not certify that each operator has been trained and evaluated in how to safely operate forklifts. Employees without training in forklift operations are permitted to operate the Nissan forklift to move pallets and material throughout the facility. Employees are exposed to struck by and struck against hazards. This violation reportedly occurred on or about October 3, 2016. To abate this violation the employer must either; 1) Train employees in forklift operations and document the following; a) Name of the operator b) Date of the training c) Date of evaluation d) Name of the trainee 2) Instruct and enforce that no employees are to operate the forklift unless they have been properly trained. A & D Wood Products, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1910.178(l)(6), which was contained in OSHA inspection number 993582, citation number 1, item number 7b and was affirmed as a final order on May 9, 2016, with respect to a workplace located at 112 Ward Street Elida, Ohio 45807.
