29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. A log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completed for 2010 in the detail as required by the regulation. a. At the facility located at 107 Heritage Drive, Pataskala, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2010, a description of work-related injuries and illnesses. For recorded injuries #2 through #16, employees were injured but a description of what caused the injury is not completed in the detail required by the standard. b. At the facility located at 107 Heritage Drive, Pataskala, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2010, a description of the specific body part injured for recorded injuries #3, #12, #13,#16, #18, #21, #22, and #23. Abatement certification required within ten days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
