29 CFR 1910.304(b)(3)(i): All 125-volt, single-phase, 15- and 20-ampere receptacles installed in bathrooms or on rooftops did not have ground-fault circuit-interrupter protection for personnel: Bathroom: the GFCI protected receptacle outlet was not operational, failing to activate when the grounding path was interrupted. The USDA Forest Service was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.304(b)(3)(i), which was contained in OSHA inspection number 957389, citation number 1, item number 3, and was affirmed as a final order on March 27, 2014, with respect to a workplace located at 3348 Alpine Blvd., Alpine, CA 91901. The USDA Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.304(b)(3)(i), which was contained in OSHA inspection number 911911, citation number 1, item number 10, and was affirmed as a final order on August 6, 2013, with respect to a workplace located at 41374 North Shore Drive, Fawnskin, CA 92333.
