29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:\r\n\r\na) Aquilar, Guillermo at 14189 St. Paul Street, Denver, CO 80238: On or before August 27, 2011, the employer allowed the use of a worn and frayed extension cord to power a Skilsaw, model HD77 serial number HD81779. The skilsaw was not grounded, but was on a working ground fault circuit interrupter.
