29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: On or about January 21, 2014, the employer did not guard the 1.5 inch rotating shaft end on the conveyor attached to Bander 600 located in the manufacturing department. PACKAGING CORPORATION OF AMERICA WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, (1910.219(c)(4)(i)), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER (893779), CITATION NUMBER 01, ITEM NUMBER 011, AND WAS AFFIRMED AS A FINAL ORDER ON (09/24/2013), WITH RESPECT TO A WORKPLACE LOCATED AT (N9090 CITY ROAD EAST TOMAHAWK, WI 54487).
