29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a) Gar Products: The Company forklift operators were not evaluated every three years in their ability to operate the powered industrial truck safely. The Company provided forklift training to their operators on 12/29/2009 and did not re-evaluate their performance on 12/29/2012. Occurred on or about 11/01/2013.
