29 CFR 1910.219(c)(2)(i): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casings enclosing shafting completely or by troughs enclosing sides and top or sides and bottom of shafting: (a) On or about September 16, 2013 the inspection revealed that employees were exposed to serious injury from a horizontal shaft that was not fully guarded. The horizontal shaft was part of pump #1 at elevation 154 located at the John Day Dam. (b) On or about September 16, 2013 the inspection revealed that employees were potentially exposed to serious injury from a horizontal shaft that was not fully guarded on the Cincinnati Bickford Radial drill located in the lower machine shop at the John Day Dam. The Army Corps of Engineers was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 682538, citation number 1, item number 2 and was affirmed as a final order on November 26, 2012, with respect to a workplace located at 8010 NW Saint Helens Road in Portland, Oregon. The Army Corps of Engineers was previously cited for a violation of this occupational safety and health standard , which was contained in OSHA inspection number 730422, citation number 1, item number 4 and was affirmed as a final order on April 24, 2013, with respect to a workplace located at The Dalles dam in The Dalles, Oregon.
