29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. Location: Location: 42 West Street, Brooklyn, NY. 5th floor, Elevator Shaft Room. a) Employees were using elevator shaft that was being energized by metalic duplex electrical boxes that the strain relief was not connected to the box. On or about 09/25/2012. The New Haven Maintenance, LLC was previously cited for violation of this Occupational Safety and Health Standard or its equivalent standard, 29 CFR 1926.405(g)(2)(iv), which was contained in OSHA inspection number 312971559, citation 1 item 1, issued on 06/15/2009, this citation become final order on 06/30/2009.
