29 CFR 1926.1053(b)(1): The employer does not ensure ladder side rails extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top. This violation was most recently observed on 05/18/12, at a jobsite at, Hooksett Street, Hampton, NH, 03842 when the side rails of the extension ladder, used to access the roof, did not extend 3 feet above the upper working surface roofing bracket where employees were working. Prior violations referred to include a violation of this Occupational Safety and Health Standard which was contained in: High & Dry Roofing, OSHA Inspection Number 332342, Serious Citation Number 01, Item Number 004, issued on 05/08/2012, with a final order date of 05/31/2012, with respect to a workplace located at 22 Lafayette Road, North Hampton, 03862. High Top Roofing, OSHA Inspection Number 306944265, Willful Citation Number 02, Item Number 002, issued on 01/05/2004, with a final order date of 01/27/2004, with respect to a workplace located at the Corner of Putnam Street and Flaherty Lane, Manchester, NH, 03101.
