The employer does not develop and implement written operating procedures that provided clear instructions for safely conducting activities in each covered process consistent with the process safety information: 29 CFR 1910.119(f)(1): The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities in each covered process consistent with the process safety information and which addressed the elements listed in 29 CFR 1910.119(f)(1)(i) through (f)(1)(iv): UPSTATE NIAGARA COOPERATIVE, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312535800, CITATION NO.1, ITEM NO.5(b), AND WAS AFFIRMED AS A FINAL ORDER ON 02/13/09, WITH RESPECT TO A WORKPLACE LOCATED AT 1730 DALE ROAD CHEEKTOWAGA, NY, 14225. A recent example of this occurred on 10/26/11, when: a) In the establishment, Employer's Standard Operating Procedures(SOPs) for Reciprocating Refrigeration Compressors RC-01 and RC-06, Effective Date: 10/26/2011, were not accurate. Both SOPs stated the presence of Pressure Relief Valves as Safety Systems. Compressor P&ID: PSM 05 and Process Hazard Analysis(PHAs) documents clearly shown that these PRVs are not installed. To abate this violation, the employer shall review their Standard Operating Procedures(SOPs) to make sure SOPs are accurate and consistent with the process safety information. Pursuant to 29 CFR 1903.19(d), the employer must submit documents describing the steps it is taking to ensure compliance, within 30 days of the date it receives this citation.
