29 CFR 1910.134(k)(6):The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: a) Omni Services, Inc., 6191 East Molloy Road, East Syracuse, NY 13057, on or about 4/14/25; The employer did not provide employees who voluntarily wore respirators, including, but not limited to, 3M 5300 half face elastomeric respirator, with Appendix D of 29 CFR 1910.134. Abatement Certification is required.
