29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. (a) On or about March 14, 2025, at 3330 E. Grange Ave., Cudahy, WI 53110, employees were accessing the roof to conduct roofing work from a ladder on the left (west side) of the home which did not extend at least three (3) feet above the upper landing surface (extended approximately 1ft past upper landing), exposing employees to a fall hazard of approximately 12 feet 6 inches. (b) On or about March 14, 2025, at 3330 E. Grange Ave., Cudahy, WI 53110, employees were accessing the roof to conduct roofing work from a ladder on the front (south) side of the home which did not extend at least three (3) feet above the upper landing surface (extended approximately less than 1ft past upper landing), exposing employees to a fall hazard of greater than 6ft. (c) On or about March 14, 2025, at 3330 E. Grange Ave., Cudahy, WI 53110, employees were accessing the roof to conduct roofing work from a ladder on the right (east) side of the home which did not extend at least three (3) feet above the upper landing surface (extended approximately 1ft past upper landing), exposing employees to a fall hazard of approximately 12 feet 9 inches. (d) On or about March 14, 2025, at 3330 E. Grange Ave., Cudahy, WI 53110, employees were accessing the roof to conduct roofing work from a ladder on the right (east) side of the home which did not extend at least three (3) feet above the upper landing surface (extended approximately less than 1ft past upper landing), exposing employees to a fall hazard of approximately 12 feet 9 inches. Everardo Servin LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1621869, citation number 1, item number 1 and was affirmed as a final order on January 12, 2023, with respect to a workplace located at 1236 Allen Rd., Oconomowoc, WI 53066. Everardo Servin LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1629767, citation number 2, item number 3 and was affirmed as a final order on April 28, 2023, with respect to a workplace located at 414 S. Silver Lake St., Oconomowoc, WI 53066. Everardo Servin LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1678064, citation number 1, item number 3 and was affirmed as a final order on December 7, 2023, with respect to a workplace located at 851 S. Wilson St., Hartford, WI 53027. Everardo Servin LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1776732, citation number 2, item number 2 and was affirmed as a final order on March 12, 2025, with respect to a workplace located at 3747 E. Bluestem Dr., Oak Creek, WI 53154.
