29 CFR 1910.119(f)(1)(i)(D): The employer did not develop and implement written operating procedures that provide clear instructions for emergency shutdown, including the conditions under which emergency shutdown is required, and the assignment of shutdown responsibility to qualified operators to ensure that emergency shutdown is executed in a safe and timely manner: (a) Throughout the facility; On or about November 20, 2024, Process Control Procedures (PCPs), including but not limited to, PCP-583 (Job C717), PCP-634 (Job C846), and PCP-732 (Job C866), and PCPs per buildings 2 and 10 did not provide clear instructions per circumstances that required immediate shutdown of the process to include failure to maintain grounding and humidity, as well as inclement weather (i.e. lightning).
