29 CFR 1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. (a) On or about April 9, 2024, located at 738 East Monroe Avenue, Hartford, WI 53027, the employer failed to ensure the extension ladder used to access the one-story garage extended at least three feet above the upper landing surface being accessed, exposing employees to a fall hazard of approximately nine (9) feet. Alexander Hernandez DBA A&A Roofing & Siding LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1593743, citation number 1, item number 3 and was affirmed as a final order on 09-28-2022, with respect to a workplace located at 1198 Olson Ave, Oshkosh, WI 54901.
