29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): Corner of Davis Ranch Parkway and Skanks Mill, San Antonio, TX, 78254: On or about September 13, 2023, and times prior thereto, employees were exposed to a cave-in hazard while installing a water main pipe valve, inside an excavation with no cave-in protection in place. The CTR Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.652(a)(1) which was contained in OSHA inspection number 1602125, citation number 1, item number 3, and was affirmed as a final order on 09-20-2022, with respect to a workplace located at 10500 Mitra Way & University Way, San Antonio, TX 78201.
