29 CFR 1926.501(b)(4)(i):Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) 345 S. Warren Street Garage, Syracuse, NY, 13202: On or about 3/28/2023 the employer did not protect employees from falling through holes while demolishing sections of the parking garage floor exposing employees to a fall distance of approximately 9 feet 2 inches. Warren Garage LLC DBA CNY Parking Corp, Warren Garage Holdings LLC, Warren Garage Management L.L.C., and United Partners, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(4)(i), which was contained in OSHA inspection number 1336936, citation number 1, item number 5 and was affirmed as a final order on 09/23/2019, with respect to a workplace located at 345 S. Warren Street, Syracuse, NY 13202.
