29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained. At an establishment located at 121 Cedar Street, in Tigerton, Wisconsin; (a) On or about July 28, 2023, in the Sawmill, an employee did not affix a personal lockout or tagout device(s) prior to performing cleaning activities under the headsaws exposing employees to accidental/inadvertent restart of the equipment. (b) On or about July 28, 2023, in the Planing/Stacking Building, employee(s) did not affix a personal lockout or tagout device(s) prior to performing cleaning activities under the stacker exposing employees to accidental/inadvertent restart of the equipment. Tigerton Lumber Company was previously cited for a violation of this Occupational Safety and Health standard which was contained in OSHA inspection number 1346228, citation number 1, item number 1 and was affirmed as a final order on January 14, 2020, with respect to a workplace located at 121 Cedar Street in Tigerton, Wisconsin.
