29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. a) Jobsite located at or near 821-25 North 13th Street, Philadelphia PA- Exterior Large Scaffold- On or about May 22, 2023, the employer did not ensure employees were provided safe means to access and egress the scaffold. Employees utilized the scaffold supports to access the 5th level of the scaffold platform, thereby exposing employees to fall hazards of approximately 26 feet. b) Jobsite located at or near 821-25 North 13th Street, Philadelphia PA- Exterior Northwest Scaffold- On or about June 9, 2023, the employer did not ensure employees were provided safe means to access and egress the scaffold. Employees utilized the scaffold supports to access the 1st level of the scaffold platform, thereby exposing employees to fall hazards of approximately 6 feet. c) Jobsite located at or near 821-25 North 13th Street, Philadelphia PA- Exterior Northeast Scaffold- On or about June 9, 2023, the employer did not ensure employees were provided safe means to access and egress the scaffold. Employees utilized the scaffold supports to access the 1st level of the scaffold platform, thereby exposing employees to fall hazards of approximately 6 feet. d) Jobsite located at or near 821-25 North 13th Street, Philadelphia PA- Inside Building Interior Scaffold- On or about June 9, 2023, the employer did not ensure employees were provided safe means to access and egress the scaffold. Employees utilized the scaffold supports to access the 3rd level of the scaffold platform, thereby exposing employees to fall hazards of approximately 12.5 feet. GTO Construction, INC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1623403, Citation Number 1, Item Number 2 and was affirmed as a final order on 3/21/2023, with respect to a workplace located at 641 N 11th St., Philadelphia, PA 19123.
