29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective. On or about May 11, 2023, employees working at the Line #1 Tipper machine, used a flexible cord with a missing ground pin to provide power to equipment including but not limited to leaf blowers used during cleanup operations.
