29 CFR 1915.12(e): The employer did not inform outside rescue teams of the hazards that the team may encounter when called to perform confined and enclosed space or other dangerous atmosphere rescue at the Employer's facility so that the rescue team can be trained and equipped: a) Barge 141: On or about May 5, 2023 and at times prior thereto, employees entered a confined space and succumbed to respiratory hazards getting light headed and had to self rescue, the employer had not established a shipyard rescue team trained in confined space rescue, although employees entered confined spaces doing ship repair work. The JAG Industrial and Marine Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1915.12(e), which was contained in OSHA inspection number 1366121, citation number 1, item number 2 and was affirmed as a final order on February 27, 2019, with respect to a workplace located at 1000 SW Altree Ln, F/V Pegasus, Toledo, OR 97391.
