Text For Citation: 02 Item/Group: 003 Hazard:
29 CFR 1910.219(c)(2)(i): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires: At an establishment located at 2099 Badgerland Drive in Green Bay, WI; employees were not protected from entanglement hazards created by exposed, rotating, horizontal shafts when: a) On or about February 1, 2023, in the Grinding Room, an employee manipulating a Contromatics valve extension handle stood on the base of nearby pump motor. The employee's leg was approximately 9 inches from the unguarded drive shaft of the motor. b) On or about January 27, 2023, on the Main Floor, employees were cleaning near and walking by the unguarded, rotating drive shaft of the 60K Cooling Tower Pump motor. c) On or about February 1, 2023, on the Main Floor, employees were cleaning around the unguarded, rotating drive shaft of the 80K First Stage Feed Pump. d) On or about January 27, 2023 through February 8, 2023, in the Grinding Room, employees retrieved ladders and shovels, and placed their coat on the guardrail next to the unguarded, rotating drive shaft on the Scrubber #4 Recycle Pump motor. Sanimax was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA inspection number 1439428, citation number 1, item number 3, and was affirmed as a final order on November 20, 2019, with respect to a workplace located at 2099 Badgerland Drive in Green Bay, Wisconsin.
