29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146: a) 550 E Front St South Thomasville, AL, 36784. Boiler house, Fuel storage area; On or about November 14, 2022, and at times prior to, the employer required employees to enter the permit confined spaces of the woodchip/bark-chip storage area without developing and implementing a written permit space entry program, exposing employees to caught-by (chain conveyor), entrapment, and engulfment hazards.
