29 CFR 1926.1203(d): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with �1926.1204 implemented at the construction site: (a) On or about November 9, 2022, at the Dry Bayou crossing beneath the Bayou Meta Canal project, employees were entering the 54-inch diameter culverts to place gasoline-engine powered water pumps to dewater the culverts. The employer did not develop and implement a written permit space entry program.
