29 CFR 1926.1053(b)(1): Where portable ladders are used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: Mike Krueger dba Altogether Roofing, LLC does not ensure the side rails of three portable aluminum extension ladders are extended at least three feet above the landing surface. Eight employees were exposed to falls from approximately 12 feet or greater from the top of the second (roof) level to the decking below while using the ladder to access the second (roof) level to perform residential roofing activities on a single-family home. This violation was most recently observed on or about June 22, 2022 at a jobsite located at 3359 Stonebrooke Ln. Maumee OH, 43537. To abate this citation, the employer must demonstrate that portable ladder safety and proper usage training is provided and being used by its employees when accessing roofs and other elevated work surfaces while using portable ladders. Mike Krueger dba Altogether Roofing, LLC was previously cited for a violation of this Occupational Safety and Health Standard (29 CFR 1926.1053(b)(1)), which was contained in OSHA Inspection #1399714 (Repeat - Serious), Citation number 2, Item number 1, and was affirmed as a final order on November 7, 2019, with respect to a workplace located at 25368 Fox Hunt Drive in Perrysburg, Ohio. Mike Krueger dba Altogether Roofing, LLC was previously cited for a violation of this Occupational Safety and Health Standard (29 CFR 1926.1053(b)(1)), which was contained in OSHA Inspection #1177422 (Repeat - Serious), Citation number 2, Item number 2c, and was affirmed as a final order on March 3, 2017, with respect to a workplace located at 10429 White Oak Drive in Perrysburg, Ohio.
