29 CFR 1910.334(a)(2)(i):Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). On or about May 3, 2022 and times prior thereto and there after, the employer did not ensure employees were not exposed to electrical hazards due to extension cords and corded equipment not being inspected prior to the job shift for physical defects, such as missing ground pins and exposed wiring on fans used in employee work areas. a) Three out of Four Pedestal Fans in Trim and Grind area were missing ground pins on May 20, 2022; b) One Pedestal Fan in the Vacuum Building was missing a ground pin and the wiring sheathing was damaged on May 20, 2022.
