29 CFR 1910.146(d)(3)(i): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including specifying acceptable entry conditions: On or about May 13, 2021, and at times prior, at the Kerr Dam, the employer does not implement acceptable levels for hazardous atmospheres. Employees entered into the inspection gallery to perform work and were exposed to hazardous conditions. Pursuant to 29 CFR 1903.19, within thirty (30) calendar days of the date of this citation, the employer must submit documentation describing the steps taken to ensure the employer implements acceptable entry conditions to protect employees from permit-required confined space hazards.
