29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about April 26, 2021, employees were exposed to a fall of approximately 24 feet while performing roofing activities on a residential home located at 9662 Diamond Ridge Cr., Canal Fulton, Ohio 44614. The employees were using an extension ladder to access the roof that was set on a Equiptor RB4000 being used as a scaffold platform and the employer did not ensure that it extended three feet above the landing area. Miller Roofing & Exteriors Limited was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFE 1926.1053(b)(1), which was contained in OSHA inspection number 1443504, citation number 1, item number 4, and was affirmed as a final order on 12/3/19, with respect to a workplace located at 2383 15th Street, Cuyahoga Falls, Ohio 44223.
