29 CFR 1910.305(c)(4): Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface: (a) Upper Building, North Wall at Building's West End - A light switch, between the overhead door and personnel door, had a broken faceplate, exposing employees to an electrical hazard, on or about April 12, 2021. D & J Pallet Services, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.305(c)(4), which was contained in OSHA inspection number 1241919, citation number 1, item number 4 and was affirmed as a final order on October 9, 2018, with respect to a workplace located at 80 Foose Ridge Road, Elliottsburg, Pennsylvania 17024. Abatement certification and documentation required within 10 days after the abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement, or other written records.
