Text For Citation: 02 Item/Group: 003 Hazard:
29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces: At a worksite located at 17562 Gum Rd, Carthage, MO: Employees were exposed to caught-in hazards. The employer is failing to protect employees by not warning them of the danger imposed by the equipment space with a sign reading DANGER-PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language would satisfy the requirement for a sign. a) This was demonstrated on January 28, 2020 when a permit space was identified on a Rework Hopper (TK-008), but contained no danger sign on the hopper. b) This was demonstrated on January 29, 2020 when a permit space was identified on a SN Reactor Tank (TK-400), but the sign was no longer legible at the entrance to the confined space. c) This was demonstrated on January 29, 2020 when a permit space was identified on a Oxidizer Storage Tank (TK-1100), but the sign was no longer legible at the entrance to the confined space.
