29 CFR 1910.147(d)(2): Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. a. On or about November 18, 2019 and at times prior, employees were exposed to machine hazards associated with moving parts and struck-by hazards during maintenance and servicing activities on various Hydraulic Presses in Department 13. The employees were changing out hydraulic valves without implementing energy control application steps as the machine was not shut down utilizing the company's established procedure for energy control for the 6-place hydraulic presses. The machine was shutdown by pressing of the E-stop and engagement of the interlocked machine doors. b. On or about November 26, 2019, and at times prior, employees were exposed to machine hazards associated with moving parts and struck-by hazards during maintenance and servicing activities in Department 13 on Hydraulic Press 3. Employees were performing a tooling repair and changeover to replace broken tooling on Press 3. The employer relied on the interlocks on the machine doors as protection for the employee while performing a task that did not afford control reliable protection. The employer did not implement energy control application steps as the machine was not shut down utilizing the company's established procedure for energy control for the 6-place hydraulic presses. The machine was shutdown by opening the interlocked doors and pressing the E-stop button inside of the doors. Tenneco Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent Standard [1910.147(d)(2)], which was contained in OSHA inspection number 1202742, citation number 1, item number 2 and was affirmed as a final order on May 26, 2017, with respect to a workplace located at 1601 Highway 49B North, Paragould, AR 72450. Tenneco Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent Standard [1910.147(d)(2)], which was contained in OSHA inspection number 1057798, citation number 1, item number 1, and was affirmed as a final order on September 28, 2015, with respect to a workplace located at 1601 HIGHWAY 49B, Paragould, AR 72450.
