Text For Citation: 02 Item/Group: 003 Hazard:
29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: 285 Locust St,. Woburn, MA. Building 7 On or about October 16, 2019, workers exposed to multiple fall hazards greater than 6-feet to a lower level or ground below were not adequately trained Instance 1: Workers cutting plywood on walking-working surface greater than 14-feet without fall protection. Instance 2: Workers unloading trusses from powered industrial truck on walking-working surface greater than 14 -feet without fall protection. Instance 3: Worker handling and installing trusses wear a personal fall arrest system, with a vertical lifeline that was not connected to anchoring device. Instance 4: Workers exposed to falls through floor holes and walking/working surface edges over 14 feet to ground without fall protection Instance 5: Workers did not install hinged ridge anchors in accordance with the manufacturer's instructions. Instance 6: Multiple workers connected their vertical lifelines at the same time to the same D-ring anchor. Instance 7: Workers connected to vertical lifelines travel almost 90-degrees from the anchor that would initiate a swing fall and strike the lower level below. The Bernardes Carpentry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1147253, citation number1, item number3 and was affirmed as a final order on August 6, 2016, with respect to a workplace located at 315 Broadway, Somerville, MA 02145
