Text For Citation: 02 Item/Group: 003 Hazard:
Under section 17(c) of the Occupational Safety and Health Act of 1970 and 29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service: Taylor Pallets & Recycling, Inc., 1030 Bankhead Hwy., Winder, GA (Jobsite) - On or about May 30, 2019, employees were required to operate the sit-down forklifts on a daily basis hauling pallet material into the shop area. The employer did not ensure that daily inspections were performed on the forklifts before placing it in service to detect any deficiencies/defects, exposing employees to struck by hazards in the following: a)Toyota sit down forklift #1 - The back-up warning alarm system and lights were inoperable and/or missing and the seat belt was inoperable/broken. b)Toyota sit down forklift #3 - The back-up warning alarm and lights were inoperable and/or missing and the horn did not work. c)Toyota sit down forklift #6 - The back-up warning alarm was inoperable and/or missing and the horn did not work. d)Toyota sit down forklift #8 - The back-up warning alarm and lights were inoperable and/or missing, the horn was inoperable, and the seat belt was inoperable/broken. The employer did not remove defective forklifts from service. The Taylor Pallets & Recycling, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(q)(7), which was contained in OSHA inspection number 1314993, citation number 1, item number 2 and was affirmed as a final order on 07/02/2018, with respect to a workplace located at 1030 Bankhead Hwy, Winder, Ga.
