29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph: a) 1404 East Palmer St., Philadelphia, PA 19125 - The employer did not ensure that a fabricated frame scaffold, on which employees performed brick masonry at heights up to 24 feet, was guyed, tied, or braced to prevent scaffold collapse when the scaffold exceeded the height to base ratio of 4:1, thus exposing employees to fall from height hazards. This condition was observed on or about April 9, 2019. Vyacheslav Leshko aka Slavik Leshko was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1218454, citation number 2, item number 3 and was affirmed as a final order on October 11, 2017, with respect to a workplace located at 827 North 26th Street, Philadelphia, PA 19130. Vyacheslav Leshko dba T&S Masonry, LLC. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 975468, citation number 1, item number 2 and was affirmed as a final order on 21 NOV 2014, with respect to a workplace located at Kimball Street and S. 23rd Street Philadelphia, PA 19146.
