29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system. a. On or about December 11, 2018, at the above addressed jobsite, employees performing spray insulation operations were exposed to falls of approximately two (2) stories created by the presence of wall openings. Rubens Spray Insulation, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1926.501(b)(1) which was contained in OSHA inspection number 1204339, citation number 1, item number 5b and was affirmed as a final order on April 25, 2017, with respect to a workplace located at 5 North Oakley Avenue, Chicago, IL 60612.
