29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a) Underground concrete vaults and steel gasoline tanks numbers 1, 2, & 3 - On or about June 12, 2018, the employer did not properly evaluate the underground tank vaults to determine if they would be permit-required confined spaces requiring appropriate measures to ensure that acceptable entry conditions were met before employees were allowed to conduct entry and attendant activities.
