29 CFR 1910.146(d)(9): The employer did not develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and/or for preventing unauthorized entry: a. On or about 1/16/2018 at the establishment located at 5911 E. Dr. Martin Luther King Junior Blvd., Tampa, FL, the employer did not implement procedures for summoning rescue and emergency services, if needed, for employees performing cleaning tasks inside permit-required chemical transporting tanks, who were exposed to a hazardous atmosphere created by carbon monoxide. An employee lost consciousness inside the tank. The Tampa Bay Tank Wash, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1021462, citation number 1, item number 7 and was affirmed as a final order on 7/21/2015 with respect to a workplace located at 5911 E. Dr. Martin Luther King Junior Blvd., Tampa, FL.
