29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the following elements and actions and were not done in the following sequence: a. Wilbert, Inc. located in Bellevue, Ohio: On or about October 23, 2017, the employer did not ensure a maintenance employee repairing a hydraulic seal on press A5 was protected from the release of potentially hazardous energy. The employee was exposed to hydraulic pressure from the press's hydraulic system and struck-by hazards from the press's robot when the press was shut down using a lockable e-stop on the control panel. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards. b. Wilbert, Inc. located in Bellevue, Ohio: On or about October 23, 2017, the employer did not ensure a maintenance employee repairing press C2 was protected from the release of potentially hazardous energy. The employee was exposed to the rotational energy of the motor, coupling, and barrel screw when the press was shut down using the electrical disconnect without applying a lockout device. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards. c. Wilbert, Inc. located in Bellevue, Ohio: On or about February 12, 2018, the employer did not ensure employees inserting and removing tooling from press 9a were protected from the release of potentially hazardous energy. The employees were exposed to crushing hazards from the press's hydraulic system and struck-by hazards from the press's robot when the press was shut down using a lockable e-stop on the control panel. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards. Wilbert, Inc. was previously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1910.147(d)(4), which was contained in OSHA inspection number 1174670, citation number 2, item number 1 and was affirmed as a final order on January 9, 2017, with respect to a workplace located at 635 Southwest St., Bellevue, Ohio. Wilbert, Inc. was previously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1910.147(d)(5)(i), which was contained in OSHA inspection number 1129402, citation number 1, item number 1d, and was affirmed as a final order on May 18, 2016, with respect to a workplace located at 635 Southwest St., Bellevue, Ohio. Wilbert, Inc. was previously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1910.147(d)(2), which was contained in OSHA inspection number 1063115, citation number 1, item number 8a and was affirmed as a final order on April 13, 2016, with respect to a workplace located at 635 Southwest St., Bellevue, Ohio.
