29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): The employer does not protect each employee in its production fabrication work tasks from the caught-in between hazards at the unguarded point of operation by the use of adequate guarding devices or other necessary means. This most recently occurred on or about May 4, 2017, at 101 North Main Street, Newark, Illinois. Employees are exposed to caught in-between amputation hazards on the SteelWeld 500 Ton mechanical press brake model no. M06-12, while hand-feeding, aligning and supporting metal parts in the unguarded point of operation for bending fabrication tasks. This employer has been cited for this practice that included: Dierzen Trailer. Co., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii) which was contained in OSHA inspection number 1125876, citation 2, item 1, issued on August 5, 2016, and became final order on August 29, 2016, with respect to a workplace located at 101 N. Main St., Newark IL 60541; and, OSHA inspection number 957315, citation 1, item 1, issued on May 23, 2014; and became final order on June 20, 2014, with respect to a workplace located at 101 N. Main St., Newark, IL 60541; and, OSHA inspection number 314040320, citation 2, item 1, issued on February 9, 2011, and became final order on or about March 7, 2011, with respect to a workplace located at 101 N.
