29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph. a) Jobsite - The employer does not ensure that the scaffolding is guyed, tied, or braced to prevent scaffold collapse when the scaffold exceeds the height to base ratio of 4:1. This violation was most recently observed at the property 827 North 26th Street, Philadelphia, Pennsylvania 19130 on or about 20 MARCH 2017 where employees were performing bricklaying operations on a scaffold that was approximately thirty (30) feet high and five (5) feet wide, with a height to width ratio of 6:1. To abate this ongoing violation, the employer must guy, tie, or brace the scaffold to prevent the scaffold from tipping or collapsing. Vyacheslav Leshko dba T&S Masonry, LLC. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 975468, citation number 1, item number 2 and was affirmed as a final order on 21 NOV 2014, with respect to a workplace located at Kimball Street and S. 23rd Street Philadelphia, PA 19146.
