29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): The employer is failing to protect employees from struck-by and crushed-by hazards associated with trenching. This was most recently documented on or about December 15, 2016 at the jobsite located at or near 507 Coleman Road in Belton, Missouri. The employer did not provide or require the use of benching, sloping, shoring or other means to ensure that employees working in excavations were protected from excavation wall failure. An employee working in the trench, which was approximately 8 to 12 feet in depth, approximately 2 to 3 feet wide and approximately 20 feet long, sustained fatal injuries when the trench wall collapsed.
